JCOH was founded in 1998 when two neighborhood associations, one from Fitchburg and one from Gardner, joined forces to collectively address the social determinants of health.
Over the years, what began with a small group of neighbors grew into a “coalition of coalitions”; people from all walks of life joined JCOH, banding together to bridge gaps and achieve together what they could not do alone.
We make it a priority to actively engage and listen to our community. We put extra effort into seeking the perspectives of those who are struggling the most and at highest risk of poor health outcomes.
We recognize, value and trust the intelligence of our community. Together, we work to craft sustainable, evidence-based initiatives focused on creating a healthier and more equitable future for all of us.
